What the research shows over time

Most human glutathione studies for skin appearance are small, short (roughly 4 to 12 weeks), and measure skin tone with instruments (a melanin index or colorimeter), not with the kind of side-by-side photos that circulate online. The results are best described as modest and inconsistent rather than transformational.

In an early randomized, double-blind, placebo-controlled trial, 60 healthy adults took 500 mg/day of oral glutathione or placebo for 4 weeks. The melanin index dropped at all six measured body sites in the glutathione group, but the difference reached statistical significance at only two of six sites (the right cheek and sun-exposed forearm), and the authors explicitly cautioned that long-term safety was not established (Arjinpathana & Asawanonda, 2010). A later 12-week trial testing lower doses (250 mg/day of the reduced or oxidized form) found melanin index and UV spots tended to be lower than placebo, with some sites also showing reduced wrinkling and no serious adverse effects reported (Weschawalit et al., 2017).

Combination approaches have been tested too. An 8-week double-blind RCT reported that pairing topical and oral glutathione produced a lower melanin index than either used alone (Wahab et al., 2021). However, a 12-week Indonesian multicenter RCT of an oral glutathione supplement (combined with vitamin C, alpha-lipoic acid, and zinc) found the reductions in skin tone and UV spots were greater than placebo but not statistically significant (Sitohang et al., 2021) — a useful reminder that “trended better” is not the same as “worked.”

A 2024 systematic review pulled these together: five randomized trials of oral glutathione (250 mg once or twice daily, or 500 mg daily) showed a significant reduction in melanin index versus placebo, but the authors rated the overall body of evidence as having a roughly equal split of low- and high-risk-of-bias studies, and concluded that intravenous glutathione is “contraindicated due to lack of efficacy and side effects” (Sarkar et al., 2024).

Clinical vs. preclinical vs. anecdotal: The skin-tone trials above are genuine human clinical data (that is the strongest tier here). Glutathione’s role as an antioxidant and its interference with melanin synthesis are supported by laboratory/preclinical work — mechanism, not proof of a cosmetic outcome. The transformation photos you find on social media and vendor pages are anecdotal: uncontrolled, unverified, and the weakest form of evidence.

Realistic expectations

The data support a small, gradual, instrument-measured shift in skin melanin index over weeks in some study populations — not a guaranteed, visible “glow-up.” Several trials failed to reach significance, effects varied site-to-site within the same person, and no study we found demonstrates durable results after supplementation stops. Individual variation is large, and baseline skin type, sun exposure, and co-interventions all confound outcomes.

This is exactly why online “before and after” photos are unreliable, especially for a research compound. Lighting, white balance, camera settings, makeup, angle, and post-processing can manufacture a “result” that no colorimeter would confirm. There is no way to verify dose, product purity, timeline, or whether the photo pair is even the same person. Treat such images as marketing, not evidence.

Oral, topical or injected: does the route change the result?

This is where the searches split — “glutathione injections before and after” is asked almost as often as the oral question — and the evidence points in opposite directions for the two routes.

  • Oral. This is where the trial evidence sits. Doses of 250 mg to 500 mg per day over 4 to 12 weeks produced small reductions in melanin index versus placebo, and a 2024 systematic review found a significant pooled reduction across five randomized trials — while rating roughly half of the included studies at high risk of bias (Sarkar et al., 2024).
  • Topical plus oral. One 8-week double-blind RCT reported that the combination gave a lower melanin index than either route used alone (Wahab et al., 2021). It is a single trial: treat it as a lead, not a conclusion.
  • Intravenous. The same 2024 review concluded that IV glutathione is contraindicated for skin lightening, citing lack of efficacy and side effects (Sarkar et al., 2024). Health regulators including the U.S. FDA and the Philippine FDA have separately warned against injectable glutathione marketed for whitening. Faster delivery did not buy better evidence here — it produced worse.

How long it took in the studies

Every human skin-tone trial identified here ran between 4 and 12 weeks, and each measured its outcome at the end rather than reporting a visible change earlier. The 4-week trial found the melanin index lower at all six measured sites but statistically significant at only two (Arjinpathana & Asawanonda, 2010). The 12-week trials reported trends that were significant in one case and not in another (Weschawalit et al., 2017; Sitohang et al., 2021). Nothing in that record supports a change within days — and nothing in it tells you what happens after you stop, because no trial we reviewed followed participants past the end of supplementation.

In the short oral trials above, glutathione was generally well tolerated, with mild and temporary side effects and no serious adverse events reported over 4–12 weeks. That is reassuring but limited: the same authors repeatedly note that long-term safety is unestablished. The picture is worse for the intravenous route — the 2024 systematic review judged IV glutathione contraindicated for skin lightening (Sarkar et al., 2024). More broadly, health regulators including the U.S. FDA and the Philippine FDA have publicly warned against injectable glutathione marketed for skin whitening, citing safety concerns and lack of approval for that use.
